Job summary

Mindoula Health’s Collaborative Care Program (CCP) in Oregon is expanding to bring on a Behavioral Health Integration Clinician – Associate Level. You will work with PCPs and psychiatric consultants to deliver targeted, short-term, evidence-based care in a supervised setting.

Responsibilities include screening and brief interventions, supporting medication adherence, patient education, and EHR documentation.

Qualifications

• Master’s degree in Social Work, Counseling, Psychology, or related field.
• Clinical licensure pathway such as CSWA or equivalent in Oregon (as applicable).
• 3+ years of experience with adult populations with chronic medical + behavioral needs.
• Experience with EMRs and telehealth workflows.

Responsibilities

• Screen/assess for mental health and substance use disorders.
• Provide short-term, evidence-based interventions (BA, PST, MI).
• Support medication adherence and monitor side effects.
• Educate and provide self-management support.
• Track clinical response and adjust care plan; refer for specialty care when needed.
• Document in EHR per workflow; participate in weekly case consultations.
