Job summary

Elevance Health seeks a Behavioral Health Case Manager I – Western New York to perform telephonic case management within the licensure scope for members with behavioral health and substance use needs.

The role is virtual and full-time, with required in-person training sessions. NY licensure is required; residency in NY is not required. You will work with members in Western New York and coordinate care with providers. A comprehensive benefits package is offered.

Qualifications

• Requires MA/MS in social work, counseling, or nursing degree with 3+ years clinical experience.
• Active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist to practice per state law.

Responsibilities

• Assess member needs using appropriate screening criteria and clinical judgment.
• Develop and monitor care plans to meet identified objectives and goals.
• Coordinate with internal and external resources to ensure access to appropriate, cost-effective care.
• Perform telephonic case management within licensure scope for behavioral health needs.
