Job summary

Qualifications

• Requires MA/MS in social work, counseling, or a related behavioral health field or a degree in nursing.
• Current active unrestricted license such as RN, LCSW, LICSW, LMHC, LPC, LMFT, LMSW, or Clinical Psychologist as applicable by state law.

Responsibilities

• Performs case management telephonically and/or by home visits within the scope of licensure.
• Uses screening criteria knowledge and clinical judgment to assess member needs.
• Develops and monitors care plans to address objectives and goals.
• Coordinates with internal and external resources and providers to meet member needs.
