Job summary

Elevance Health is seeking a Behavioral Health Case Manager I to serve Richmond, VA, with telephonic and/or home-visit duties within licensure scope. The role supports members with behavioral health/substance use needs, coordinating care and developing plans.

Minimum requirements include an MA/MS in social work or related field (or nursing) and an active license (RN/LCSW/LPC/LMFT/PhD). Salary ranges from $64,240 to $96,360, with comprehensive benefits and a hybrid/remote work approach.

Qualifications

• Requires MA/MS in social work, counseling, or a related behavioral health field or a degree in nursing, and minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment.
• Current active unrestricted license such as RN LCSW (as applicable by state law and scope of practice) LPC (as allowed by applicable state laws) LMFT or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States required.

Responsibilities

• Assess member needs using screening criteria knowledge and clinical judgment.
• Develop, implement, and monitor a care plan addressing objectives identified in assessment.
• Coordinate with internal and external resources to meet member needs and collaborate with providers.
