Qualifications

Responsibilities

• Respond to more complex cases and account specific requests.
• Use screening criteria knowledge and clinical judgment to assess member needs.
• Conduct assessments to identify individual needs and develop care plans.
• Monitor and evaluate effectiveness of care plans and modify as needed.
• Support member access to quality and cost-effective care.
• Coordinate with resources to meet member needs and collaborate with providers.
• Serve as a resource to other BH Case Managers.
• Participate in cross-functional team projects and initiatives.
