Qualifications

• Active WA State license to conduct assessments independently.
• 3 years’ post-licensure direct care experience.
• 1 year post-licensure case management experience.

Responsibilities

• Engage members with mental health/substance use disorders and build trust for dialogue.
• Conduct initial holistic health assessments identifying social determinants of health (SDOH) and cultural factors.
• Develop member-centric case management plans with caregivers and providers to set goals.
• Monitor and update the effectiveness of the plan toward goals.
• Coordinate communication among the health care team to ensure appropriate care.
• Provide interventions to remove barriers and promote positive behavior change using motivational interviewing.
• Maintain accurate documentation meeting accreditation, contractual, and legal requirements.
