Independence Blue Cross is seeking a Behavioral Health Triage Case Manager. This fully remote role supports members in the Tri-State area (DE, NJ, PA) with crisis intervention, care navigation, and coordination with care teams.

You will assess needs, provide information, and facilitate access to behavioral health services, while promoting evidence-based practices and coordinating with physicians and the plan.

Formación

• Active PA RN license required.
• BSN or LSW/LCSW/LPC credentials accepted.

Responsabilidades

• Provide telephone triage and crisis intervention for at-risk members.
• Coordinate care plans with members and health care teams.
• Deescalate crises and provide brief supportive counseling.
• Match members with behavioral health and substance-use providers.
• Identify resources to help members access services and benefits.
