Job summary

Independence Blue Cross in the Tri-State Area is seeking a Behavioral Health Case Manager to coordinate care for members with behavioral health needs. This fully remote role requires navigating SDOH, developing individualized care plans, and collaborating with healthcare teams.

You will engage members by phone and provide guidance, education, and advocacy to support treatment decisions, improve health outcomes, and maximize benefits.

Qualifications

• Active PA Registered Nurse licensure required.
• Experience in an integrated care setting.
• Strong assessment and engagement skills; telephonic/virtual work.

Responsibilities

• Telephonically engages members to identify barriers to care related to Social Determinants of Health (SDOH).
• Develops care plans with identified strengths and needs; provides follow-up as clinically appropriate.
• Serves as liaison within the member’s health care team for complex behavioral health cases.
• Coaches members to access online, telephonic and community resources; optimizes benefits and service coordination.
• Provides clinical assessment, intervention and support to members via inbound calls.
