Job summary

The Behavioral Health Case Manager/CBRS Specialist will provide in-home and community-based services, coordinating with clinicians and care teams to support clients in achieving functional goals.

Qualifications

• Bachelor's degree or higher in a health or human services field is required.
• Ability and willingness to participate in ongoing clinical supervision.
• Valid driver's license and reliable transportation for regular travel between client locations.

Responsibilities

• Provide community-based case-management and CBRS/skills-building services.
• Coordinate with treating clinicians to integrate CBRS with ongoing treatment.
• Complete accurate documentation and progress notes per payer and HIPAA requirements.
• Monitor client progress toward goals and communicate concerns to the supervising clinician.
• Maintain professional boundaries and deliver person-centered, culturally responsive services.
