Job summary

Munson Healthcare seeks a Behavioral Health Care Manager/Nurse Navigator for the South Region AMB Clinics. The role is primarily remote with some travel to clinic sites and meetings, Monday–Friday, 8 AM–5 PM, 40 hours per week.

The BHCM supports the Collaborative Care model by coordinating behavioral health services within primary care and engaging patients in care plans. You will partner with PCPs and psychiatric consultants, perform proactive follow-ups, and document progress using

Qualifications

• Active RN licensure in good standing.
• Experience in care management, behavioral health, or primary care settings.
• Strong communication and patient-engagement skills.
• Ability to work remotely with independent initiative.

Responsibilities

• Provide care coordination for patients with depression and anxiety within CoCM.
• Collaborate with PCPs and Psychiatric Consultants to ensure timely, coordinated care.
• Utilize standardized measures to monitor progress and guide treatment decisions.
• Document notes accurately and share updates with the care team.
• Support development and optimization of Collaborative Care workflows across clinics.
