Job summary

Optima Medical in Arizona seeks a Behavioral Health Care Coordinator (BHCC) to support members with behavioral health needs through care coordination, case management, and resource navigation. You will engage with members, coordinate care across providers, and help ensure access to appropriate services and support.

BHCCs provide education, encourage treatment plan adherence, and connect patients to behavioral health resources, without delivering licensed clinical treatment.

Qualifications

• Master's Degree in any field is required.
• Bachelor's Degree in Psychology, Social Work, Human Services, Public Health, Behavioral Health or related field is required.
• Bachelor's Degree in any field with at least 1 year of relevant experience in behavioral health, healthcare, care coordination, case management, or human services is required.
• High School Diploma/GED with four or more years of relevant experience in behavioral health, healthcare, care coordination, case management, or human services is required.

Responsibilities

• Conduct outreach, engagement, and follow-up with members.
• Assess patient needs and identify barriers to care.
• Coordinate services between patients, providers, and community resources.
• Develop and monitor individualized care plans.
• Facilitate referrals and support appointment and treatment adherence.
• Support transitions of care, including hospital admissions and discharges.
• Maintain timely, accurate, and compliant documentation.
• Collaborate with interdisciplinary teams to improve patient outcomes.
• Escalate clinical, safety, or crisis concerns to licensed staff.
• Provide education and connect patients to behavioral health, medical, and social resources.
