Job summary

CareStar, Inc. is seeking an Assessment Specialist Flex in Ohio to assess eligibility, perform safety assessments, and connect individuals with essential services. This remote with field visits role supports community-based care coordination for the Ohio Home Care program.

You will document interactions, use assessment tools, and refer clients to community resources while upholding CareStar policies and patient confidentiality. Strong knowledge of Medicare/Medicaid is a plus.

Qualifications

• Active license as RN, LSW, LISW or LPN.
• 12 months experience in home and community-based services within the past 5 years.
• Knowledge of Medicare, Medicaid, Home Health Care and Ohio Home Care waiver programs desirable.
• Adheres to the CareStar Rule in performance of job responsibilities.
• Understands and complies with CareStar Policies and Procedures.

Responsibilities

• Completes visits to assess for eligibility for specified program and conducts safety assessments, in compliance with CareStar and department guidelines.
• Documents all applicant and case-related contacts and interventions in applicants record per agency and CareStar policy, including standards of content and timeliness.
• Processes specific Program Eligibility Assessment Tool (PEAT) or other tools and all related documents per agency and CareStar policy.
• Provides assistance with community resource referrals as appropriate.
• Identifies and reports high-risk situations as required per agency and CareStar policy.
• Adhere to the CareStar Rule in performance of job responsibilities.
• Understand and comply with CareStar Policies and Procedures.
• Maintain confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
• Follow the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
