Job summary

CareStar, Inc. is seeking an Assessment Specialist Flex to support the Ohio Home Care program. The role involves conducting visits to assess eligibility, developing personalized care plans, and connecting individuals with essential services across Hamilton, OH and surrounding areas.

You will work within a mission-driven team focusing on improving lives through coordinated care. The position requires an active RN/LSW/LISW/LPN license and at least a year of home and community-based services

Qualifications

• Active license as RN, LSW, LISW or LPN.
• 12 months experience in home and community-based services in the past 5 years.
• Knowledge of Medicare, Medicaid, Home Health Care and Ohio Home Care waiver programs desirable.
• Adheres to the CareStar Rule in performance of job responsibilities.
• Understands and complies with CareStar Policies and Procedures.

Responsibilities

• Completes visits to assess eligibility and conducts safety assessments.
• Documents contacts and interventions in the applicant's record per policy.
• Processes PEAT or other tools and related documents per policy.
• Provides assistance with community resource referrals.
• Identifies and reports high-risk situations as required.
• Maintain confidentiality of patient information per policy.
• Follow Acceptable Use Policy for CareStar systems.
