Job summary

CareStar, Inc. seeks an Assessment Specialist Flex to assess individuals, develop care plans, and connect them with essential services within the Ohio Home Care program.

You will conduct eligibility visits, complete safety assessments, and document findings while adhering to policy. This remote-friendly role supports field visits across Montgomery County and surrounding areas, with a focus on improving lives through coordinated care.

Qualifications

• Active professional license (RN, LSW, LISW, or LPN) is required.
• 12 months of home and community-based services experience in the last 5 years.
• Knowledge of Medicare, Medicaid, Home Health Care and Ohio Home Care waiver programs desirable.
• Adheres to CareStar policies and procedures.
• Understands and complies with CareStar Rules.

Responsibilities

• Visits to assess eligibility for the specified program and conduct safety assessments per policy.
• Document all applicant contacts and interventions in the record per policy.
• Process Program Eligibility Assessment Tool (PEAT) and related documents per policy.
• Assist with community resource referrals as appropriate.
• Identify and report high-risk situations per policy.
• Maintain patient confidentiality and comply with information security policies.
