Job summary

Encompass Health Rehabilitation Hospital of Abilene is seeking a Case Manager for a full‑time, Monday–Friday role (8‑5). The position requires at least 2 years of Case Management experience, a current license, and CCM/ACM certification within two years of entry.

PRN options may be available, with competitive compensation based on experience and certifications. You will coordinate patient care from admission through post‑discharge, lead interdisciplinary plan‑of‑care decisions, and ensure timely

Qualifications

• Must independently complete an assessment within the scope of practice.
• Active license in the relevant discipline (e.g., RN or other).
• CCM or ACM certification required or must be obtained within two years of entry.
• Two years of rehabilitation Case Management experience preferred.

Responsibilities

• Manage resources and coordinate patient care from admission to post-discharge.
• Oversee interdisciplinary plan-of-care decisions.
• Coordinate tentative discharge plans and contingency plans with the team.
• Conduct and document case management assessments and addendums.
• Maintain compliance with regulations, company policies, and payor requirements.
• Participate in utilization review and ACT meetings as needed.
