Qualifications

• Must independently complete an assessment within the scope of practice of the discipline.
• Active license in the state of practice is required.
• CCM or ACM certification required or must be obtained within two years of hire.
• Minimum of nursing or allied health licensure for the role.

Responsibilities

• Coordinate patient care from admission to post-discharge with an interdisciplinary team.
• Coordinate discharge planning and contingency plans with the team.
• Document case management activities and ensure regulatory compliance.
• Monitor patient experience and payer-related requirements.
• Participate in weekly team conferences and treatment plan modifications.
