Job summary

Encompass Health is hiring a Case Manager in Abilene, TX to coordinate patient care from admission to post-discharge. The role emphasizes interdisciplinary collaboration, discharge planning, and ensuring compliant documentation and communication with payors.

The position requires a valid license (RN preferred) and CCM/ACM eligibility within two years. Full‑time schedule is typical, with patient-centered care across our rehabilitation network.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice.
• Must hold an active license if required for one's discipline in the state.
• CCM or ACM certification required OR must be obtained within two years of entry.

Responsibilities

• Guide treatment plans with the interdisciplinary team.
• Coordinate tentative discharge plan and contingency plans.
• Participate in planning for and execution of patient discharge experience.
• Monitor patient experience and service appropriateness.
• Facilitate weekly team conferences and coordinate plan modifications.
• Complete case management addendums and required documentation.
• Maintain knowledge of regulations, policies, and department operations.
• Review case management reports and plan actions.
• Understand payor requirements and recertification needs.
• Attend ACT meetings to identify trends and reduce ACTs.
• Meet with patient/family within 24 hours of admission.
• Educate patient/family on rehabilitation and the Case Manager role.
• Schedule and facilitate family conferences as needed.
• Assist with timely resource procurement to avoid discharge delays.
• Monitor orthotics/prosthetics ordering compliance.
• Make referrals and document to post-discharge providers/physicians.
• Ensure accuracy of discharge and payor information.
• Participate in utilization review: data collection and resolution actions.
• Participate in case management on-call schedule.
