Encompass Health is seeking a Case Manager to coordinate patient care from admission to post-discharge. You will work with an interdisciplinary team to develop and adjust treatment plans, ensure timely discharge planning, and monitor quality metrics to improve patient outcomes.

Qualifikationen

• Must be qualified to independently complete an assessment within the scope of practice of the discipline.
• Active license if licensure is required by state.
• CCM® or ACM™ certification required or must be obtained within two years of entry.

Aufgaben

• Work with interdisciplinary team to guide treatment plans based on patient needs and preferences.
• Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
• Participate in planning for and execution of patient discharge experience.
• Monitor patient experience for quality, timeliness, service appropriateness, payors and expectations.
• Facilitate weekly team conferences and coordinate all treatment plan modifications.
• Complete case management addendums and required documentation.
• Review case management reports and plan actions based on key indicators.
• Understand contract levels, exclusions, payor requirements and recertification needs.
• Attend ACT meetings to identify trends and reduce ACTs.
• Meet with patient/family within 24 hours of admission; assess goals within 48 hours.
