This role emphasizes compassionate care within a growing national rehabilitation leader, with focus on discharge planning, resource management, and compliance with regulations and payor requirements. Prior rehab experience preferred and CCM/ACM certification targeted.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice.
• Active license in the state; CCM/ACM certification required or to be obtained within two years.
• CCM or ACM certification must be obtained within two years of entry into the Case Manager II position.

Responsibilities

• Guide treatment plans with an interdisciplinary team.
• Coordinate tentative discharge plans and contingencies.
• Participate in discharge planning and patient education.
• Monitor patient experience and service quality.
• Attend weekly team conferences and update care plans.
• Complete case management addendums and documentation.
• Maintain knowledge of regulations and department procedures.
• Review case management reports and act on KPIs.
• Understand payor requirements and recertification needs.
• Attend ACT meetings to identify trends and reduce ACTs.
• Meet patient/family within 24 hours of admission; plan and document goals.

Education

Bachelor's degree

CCM/ACM certification within 2 years
