Encompass Health is seeking a Clinical Case Manager to coordinate patient care from admission through post-discharge, working with an interdisciplinary team in a leading national inpatient rehabilitation network. This role focuses on developing discharge plans, ensuring timely access to resources, and maintaining compliant documentation.

The ideal candidate holds a health-related BA/BS, is eligible for CCM/ACM certification (or committed to obtaining it within two years), and has strong

Competenze

• Must be able to independently complete assessments within the scope of practice.
• CCM® or ACM™ certification required or to be obtained within two years of entry.
• Bachelor’s degree required for all other eligible licensed/certified health care professionals; graduate degree preferred.

Mansioni

• Coordinate patient care with interdisciplinary team from admission to post-discharge.
• Establish tentative discharge plans and contingency plans with the team.
• Educate patient and family on rehabilitation and the Case Manager role; establish a communication plan.
• Participate in utilization reviews and review case management reports for improvement actions.
• Complete case management addendums and ensure accurate documentation in the patient record.
