Encompass Health Corporation seeks a Case Manager to coordinate patient care from admission through post-discharge, guiding treatment plans with an interdisciplinary team. You will develop discharge strategies, monitor patient experience, and ensure timely documentation and compliance.

• Must be able to independently complete assessments within the scope of practice.
• CCM or ACM certification required or must be obtained within two years.
• 2 years of rehabilitation experience preferred.

Responsabilidades

• Coordinate patient care from admission to post-discharge with interdisciplinary team.
• Establish tentative discharge plan and contingency plans.
• Participate in planning for and execution of discharge experience.
• Monitor patient experience and service quality, timeliness, and payor requirements.
• Facilitate weekly team conferences and modify treatment plans as needed.
• Complete case management addendums and required documentation.
• Maintain knowledge of regulations, policies, and department operations.
• Review case management reports and plan actions based on indicators.
• Attend ACT meetings to identify trends and reduce ACTs.
• Meet with patient/family within 24 hours of admission.
• Assess goals and complete addendum within 48 hours of admission.
• Educate patient/family on rehabilitation and case manager role; establish communication plan.
• Schedule and facilitate family conferences as needed.
• Assist patient with resources to avoid discharge delays.
• Monitor orthotics/prosthetics ordering and payment compliance.
