Job summary

Creative Health Solutions, LLC in Winter Haven, FL is seeking a compassionate Rehabilitation Tech for our Skilled Nursing Facility with Assisted Living. The Rehab Tech will provide supportive services under the Rehab Director and assist with patient transport, treatment preparation, and daily coordination with the nursing staff.

Qualifications

• Minimum high school education or equivalent.
• One year health care experience preferred.
• One year office experience preferred.
• Certified Nursing Assistant preferred.
• Proficient with basic computer skills.
• Strong organizational skills to manage multiple projects.
• Creativity and ability to work independently.

Responsibilities

• Transports patients as directed.
• Assists therapists with treatment.
• Coordinates scheduling with the nursing staff.
• Prepares patients, treatment areas, equipment and supplies for treatment.
• Maintains awareness of change in patient census and programs.
• Follows established procedures pertaining to the care of equipment and supplies including completion of cleaning logs.
• Ensures that adequate office supplies and supplies for treatments are maintained.
• Completing forms and records, making copies, filing forms, obtaining necessary signatures and maintaining orderly and accurate patient records including completion of Triplecheck forms.
• Assists with patient and or staff schedule on a daily basis.
• Runs and analyzes reports on a daily, weekly, and monthly basis to assist in department operations, compliance, and patient management procedures and communicates information to appropriate individuals.
