Job summary

Key Rehab is seeking a Full Time Program Coordinator (PT/PTA/OT/OTA/SLP) to join our SNF team. This role requires completion of an accredited PT, OT, or SLP program, active state licensure, and CCC-SLP certification. CPR and a valid driver license are also required.

The coordinator will liaise with community staff, minimize administrative burdens, and coordinate staffing and scheduling to meet performance indicators under regional supervision.

Qualifications

• Education: Completion of an accredited PT, OT, or SLP program.
• Licenses/Certificates: Current PT, OT, or SLP license to practice in the state(s) assigned.
• CCC-SLP certification required.
• CPR certification as required by community.
• Valid driver license and car insurance as required by state law.

Responsibilities

• Serve as liaison between Key Rehab and community staff to minimize administrative burden and support clinicians.
• Coordinate staffing and daily scheduling to meet KPIs under the direction of a Regional Supervisor.
