Overview

Key Rehab is looking for a Program Coordinator (OT or PT) to join our team in providing excellent care in the Falls City, NE area in a SNF setting.

Education and Experience

• Completion of an accredited PT, OT, or SLP program.
• Prior experience in long‑term care setting and service coordination.

Licenses and Certifications

• Current PT, OT, or SLP license to practice in the assigned state(s).
• Certificate of Clinical Competence in Speech‑Language Pathology (CCC‑SLP).
• CPR certification as required by community.
• Valid driver license and car insurance as required by state law.
• Other items as required by federal, state and local standards.

Preferred Qualifications

• Strong organizational and problem‑solving skills.
• Flexibility and team collaboration.
• Exceptional communication skills.
• Experience with geriatric population.
• Knowledge of Medicare billing regulations.

Responsibilities

In addition to providing therapy under their disciplinary scope of practice (OT, PT, or SLP), the Rehab Coordinator serves as the liaison between Key Rehab and community staff. The position is instrumental in minimizing administrative burden and providing support to allow clinicians to deliver therapy services. The role also coordinates staffing and daily scheduling to meet key performance indicators and expectations under the direction of a Regional Supervisor.
