Job summary

Encompass Health Corporation is seeking a Case Manager to manage patient care from admission through post‑discharge, coordinating with an interdisciplinary team to develop and execute care plans. The role emphasizes CCM/ACM certification and clinical expertise in rehabilitation.

You will guide treatment plans, participate in discharge planning, and educate patients and families about rehabilitation goals while ensuring compliant documentation and payor communications.

Qualifications

• Must be able to independently complete assessments within scope of practice.
• Active license for discipline per state requirements if licensure is required.
• CCM/ACM certification required or to be obtained within two years of entry.
• Bachelor's degree required for all licensed or certified health care professionals; graduate degree preferred.

Responsibilities

• Coordinate patient care from admission to post‑discharge.
• Lead and participate in interdisciplinary care planning and conferences.
• Develop tentative discharge plans and contingency plans with the team.
• Educate patient and family on rehabilitation goals and care plans.
• Ensure accurate discharge documentation and payor information in the chart.
• Engage in utilization review data collection and trend analysis.
