Qualifications

• Licensed or certified professional eligible to independently assess within their discipline.
• CCM® or ACM™ certification required or to be obtained within two years of placement.

Responsibilities

• Coordinate patient care from admission to post-discharge.
• Lead discharge planning and contingency planning with interdisciplinary team.
• Document case management actions and addendums.
• Monitor compliance with regulations and payor requirements.
• Attend care conferences and support timely patient discharge.
