Encompass Health Corporation is seeking an experienced Case Manager to coordinate patient care from admission through post-discharge. You will lead interdisciplinary care planning and ensure timely, compliant discharge planning in a nationwide rehabilitation setting.

The role emphasizes clinical expertise, regulatory knowledge, and seamless teamwork with physicians, therapists, and care teams while delivering compassionate, high-quality outcomes.

Qualifications

• Must be able to independently complete assessments within the scope of practice.
• CCM® or ACM™ certification required or must be obtained within two years of placement.

Responsibilities

• Coordinate patient care from admission to post-discharge.
• Lead interdisciplinary plan-of-care decisions and team conferences.
• Participate in discharge planning and post-discharge referrals.
• Monitor patient experience and ensure timely, appropriate care.
• Maintain documentation and regulatory compliance.
