Job summary

Encompass Health Corporation is seeking a Case Manager to coordinate patient care from admission through post-discharge, working with an interdisciplinary team to optimize rehabilitation outcomes.

You will develop discharge plans, educate patients and families, and ensure compliant documentation and payor interactions while contributing to a supportive, patient-centered care environment.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice.
• CCM® or ACM™ certification required OR must be obtained within two years of entry.
• RN licensure with BSN preferred for nursing track; others require a bachelor's degree.

Responsibilities

• Coordinate patient care from admission to post-discharge with an interdisciplinary team.
• Develop tentative discharge plans and contingency plans with care teams.
• Educate patients and families on rehabilitation goals and services.
• Ensure accurate documentation and compliance with payor requirements.
• Attend meetings and review case management data for process improvements.
