Qualifications

• Licensure in the state where the hospital resides is preferred.
• Discipline of the case manager may include OT, PT, RN, RRT, SLP, or LSW.
• Current BLS certification is required.

Responsibilities

• Reviews inpatient admission records to ensure proper utilization of hospital resources.
• Assess discharge needs and coordinate discharge plan.
• Coordinate care with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
