Job summary

PAM Health Rehabilitation Hospital of Overland Park seeks a Case Manager to coordinate interdisciplinary care for a defined patient population across inpatient settings. You will assess discharge needs, plan and coordinate discharges, and collaborate with the care team to optimize outcomes and payor relations.

Ideal candidates have 3–5 years of inpatient experience in acute, IRF, or LTACH settings and hold state licensure with BLS certification.

Qualifications

• Licensed state professional in preferred discipline with BLS certification.
• 3–5 years inpatient experience in acute/IRF/LTACH preferred.
• Interdisciplinary teamwork and effective communication skills required.

Responsibilities

• Coordinate the interdisciplinary care for a designated patient population.
• Review inpatient admission records to ensure appropriate level of care.
• Assess discharge needs and coordinate the discharge plan.
• Work closely with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
