PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Case Manager to coordinate care across disciplines for a defined patient population in our Richardson, TX location. You will review inpatient records to ensure appropriate utilization and determine admissions for the correct level of care.

You will work with nurses, therapists, and social workers to identify discharge needs and coordinate the discharge plan while maintaining and strengthening payor relationships.

Formación

• Licensure in the state where the hospital resides is preferred.
• Disciplines may include OT, PT, RN, RRT, SLP, or social worker.
• Current BLS certification required.
• 3–5 years inpatient experience, preferably in acute, IRF, or LTACH.

Responsabilidades

• Coordinate care for a designated patient population with interdisciplinary team.
• Review inpatient admission records to ensure appropriate level of care and discharge planning; maintain payor relationships.
