Encompass Health is recruiting a Case Manager in Las Vegas to coordinate care from admission through post-discharge, guiding interdisciplinary plan-of-care decisions and ensuring timely, quality patient outcomes.

The role requires active licensure, CCM/ACM certification (or within two years), and 2 years of rehab experience, with opportunities for professional growth in a nationally recognized rehabilitation leader.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice of the discipline.
• Active license in the applicable state is required.
• CCM® or ACM™ certification required OR must be obtained within two years of starting the role.
• 2 years of rehabilitation experience preferred.

Responsibilities

• Coordinate patient care from admission to discharge with interdisciplinary team.
• Lead and participate in treatment plan decisions and discharge planning.
• Document case management activities and addendums in patient records.
• Monitor quality indicators, payor requirements, and regulatory compliance.
• Attend case management meetings and on-call coverage as needed.
