Job summary

Encompass Health is seeking a Case Manager to coordinate patient care from admission to post-discharge. The role involves overseeing interdisciplinary plan-of-care decisions, discharge planning, and ensuring regulatory compliance.

You will work with an integrated team to optimize patient outcomes across the continuum of care. The position requires licensure (RN or equivalent), CCM/ACM certification or readiness to obtain within two years, and a bachelor’s degree with a graduate degree preferred

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice of the discipline (e.g., RN, SW, OT, PT, ST).
• CCM® or ACM™ certification required or must be obtained within two years of placement.
• Bachelor's degree is required; graduate degree preferred for all eligible licensed professionals.

Responsibilities

• Coordinate patient care from admission to post-discharge.
• Oversee interdisciplinary plan-of-care decisions and discharge planning.
• Maintain compliance with regulations and company policies; document all case management activities.
