Qualifications

• Bachelor's degree preferred; Associate degree required in nursing.
• Registered Nurse license required.
• BLS certification required on hire.
• De-Escalation training within 6 months of hire.
• Driver's license required.
• Experience in planning and implementation of client care.
• Ability to travel to client residences in varying environments.

Responsibilities

• Complete patient assessments, formulate and oversee care plans, and adjust based on needs.
• Perform nursing acts consistent with standards and scope of practice.
• Provide care, counseling, health teaching, and use Teach Back.
• Administer medications and IV therapy as prescribed.
• Supervise, delegate, and mentor other personnel and students.
• Report to chain of command and participate in performance improvement initiatives.
• Ensure safety and confidentiality of patient information.
• Participate in discharge planning and transitions of care.
• Advocate for patients and document pertinent findings in medical records.
