Qualifications

• RN licensure required.

• Associate's Degree required.
• Bachelor's Degree preferred.
• Demonstrated nursing skills and clinical judgment.
• Ability to collaborate with physicians and multidisciplinary teams.

Responsibilities

• Completes patient assessments, formulates plan of care, oversees implementation of plan of care.
• Oversees nursing acts consistent with standards of practice.
• Assists with discharge planning and transition of care.
• Maintains accurate documentation in the medical record.
• Provides patient and family education and support.
