Job summary

Trilogy Health Services, LLC in Gahanna, OH is seeking a Registered Nurse (RN) to provide direct clinical care and serve as a Team Leader to patient care staff.

The RN will lead a team of direct care providers in medications, treatments, documentation, family teaching, and care planning, ensuring care conferences comply with campus policies. Nursing process use and continuity from admission to discharge are essential.

Qualifications

• Must hold and maintain a current, valid state RN license.
• Current CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure medications, treatments, documentation, family teaching, care planning and patient care conferences align with Health Campus policies.
• Use the nursing process to deliver care and ensure continuity from admission through discharge and return to family and community.
• Visit residents daily to assess status and notify the charge nurse, physician, or Director of Nursing if a resident changes condition.
• Identify problems and emergencies and initiate lifesaving measures when no physician is present.
• Perform administrative duties such as completing medical forms, reports, evaluations, and charting as needed.
