Job summary

Trilogy in Ottawa, Ohio is seeking a Registered Nurse (RN) to provide direct, quality clinical care and serve as a Team Leader to patient care staff. You will lead a team of direct care providers, ensure medications and treatments are carried out, document accurately, and support families through care planning and patient conferences.

This role supports admission through discharge and requires maintaining an RN license and CPR certification.

Qualifications

• Must hold and maintain an active RN license.
• CPR certification required.
• Experience leading direct care teams is preferred.

Responsibilities

• Lead a team of direct care providers to ensure medications and treatments are executed, documentation, family teaching, care planning and patient care conferences in compliance with policies.
• Utilize the nursing process in delivering patient care and ensure continuity of care from admission through discharge and return to family and community.
• Visit residents on assigned unit daily to observe and evaluate each resident’s status and notify the charge nurse or physician when there is a change in condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc.
