Job summary

Trilogy Health Services in Leipsic, OH is seeking a Registered Nurse (RN) to provide direct, quality clinical care and to serve as a Team Leader for patient care staff on the unit.

The role focuses on leadership in medication administration, care planning, and ensuring continuity of care from admission through discharge, with daily resident visits and proactive response to changes in condition.

Qualifications

• Must hold and maintain a current, valid state RN license.
• Current, valid CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure proper medications, treatments, documentation, family teaching, care planning and patient care conferences according to policies.
• Utilize the nursing process to deliver care and ensure continuity from admission to discharge and return to family/community.
• Visit residents daily to observe status and notify the charge nurse/physician when changes occur.
• Identify problems and emergencies and initiate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing forms, reports, evaluations, and charting.
