Job summary

Select Home Health is seeking a skilled and compassionate Registered Nurse to provide in-home nursing care across central Indiana. This role emphasizes patient-centered care, adherence to physician plans of care, and thorough documentation including OASIS, with ongoing patient and caregiver education.

The ideal candidate holds an active RN license, at least 1 year of nursing experience, and a valid driver's license to travel between patients' homes.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current Registered Nurse license for the practicing state.
• Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience preferred.
• Valid driver’s license, reliable auto insurance, and personal transportation.
• Current up to date TB test.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete required documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications, disease processes, and in-home care.
• Coordinate patient services and supervise LPNs/HHAs as needed.
• Communicate changes in patient condition to physicians and care teams.
• Maintain compliance with regulations and agency policies.
