Select Home Health is seeking skilled Registered Nurses to provide in-home nursing care in West Lafayette, Indiana and surrounding areas. This role focuses on delivering high-quality, personalized care while promoting patient independence and overall well-being.

You will perform patient assessments, implement physician-ordered plans of care, complete required documentation (OASIS), educate patients and caregivers, and coordinate services with LPNs/HHAs while maintaining regulatory compliance.

Qualifikationen

• Graduate of an accredited school of professional nursing (required).
• Current Registered Nurse license in the state of Indiana (required).
• Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience preferred.
• Valid driver’s license, reliable auto insurance, and personal transportation (required).
• Current up to date TB test.
