Job summary

Select Home Health & Hospice is seeking a skilled Registered Nurse to provide in-home nursing care across multiple patients per day. You will assess, educate, coordinate services, and document physician-ordered plans of care, ensuring high quality, patient-centered outcomes.

As a clinician-led home health provider, we value compassionate care, regulatory compliance, and effective team communication. A valid RN license and drivers license are required for Indiana practice.

Qualifications

• Graduate of an accredited school of professional nursing (required).
• Current Registered Nurse license for the practicing state (required).
• Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience preferred.
• Valid driver’s license, reliable auto insurance, and personal transportation (required).
• Current up to date TB test.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete required documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications, disease processes, and in-home care.
• Coordinate patient services and supervise LPNs/HHAs as needed.
• Communicate changes in patient condition to physicians and care teams.
• Maintain compliance with regulations and agency policies.
