Job summary

Human Touch Home Health in Montgomery County, Maryland, seeks an experienced Registered Nurse (RN) for PRN, home-based visits across Bethesda, Silver Spring, Rockville and surrounding communities. Join a team that emphasizes one-on-one patient care, independence, and flexible scheduling to fit your availability.

As an RN, you will perform assessments, develop care plans, educate patients and caregivers, and coordinate with therapists and physicians.

Qualifications

• Active Maryland RN license required.
• Valid driver's license and reliable transportation.
• Strong clinical assessment, critical thinking, and documentation skills.
• Home health experience preferred but not required.
• OASIS experience preferred.
• Experience with WellSky Home Health is a plus.
• Passion for delivering exceptional patient-centered care.

Responsibilities

• Perform comprehensive nursing assessments and skilled visits.
• Complete SOC, ROC, Recertification, and Discharge assessments.
• Develop and implement individualized plans of care.
• Provide skilled nursing services, including medication management, wound care, disease management, IV therapy as appropriate, and patient education.
• Monitor patient progress and communicate changes with physicians and the interdisciplinary care team.
• Coordinate care with therapists, social workers, and home health aides.
• Complete timely and accurate clinical documentation using WellSky.
• Educate patients and caregivers to promote safety, independence, and improved health outcomes.

What You'll Do
