Job summary

Human Touch Home Health in Virginia is seeking a registered nurse (RN) to join our team serving patients in rural communities around Fredericksburg, including King George, Caroline, Orange, Locust Grove, and nearby areas. This PRN role offers flexibility, independence, and the opportunity to make a meaningful impact in underserved communities.

As a Home Health RN, you will provide skilled, individualized care, perform assessments, develop plans of care, administer treatments, educate patients

Qualifications

• Active Virginia RN license required.
• Valid driver's license and reliable transportation.
• Strong clinical assessment, critical thinking, and documentation skills.
• Home health experience preferred but not required.
• OASIS experience preferred.
• Experience with WellSky Home Health is a plus.
• Passion for delivering exceptional patient-centered care.

Responsibilities

• Perform comprehensive nursing assessments and skilled visits.
• Complete SOC, ROC, recertification, and discharge assessments.
• Develop and implement individualized plans of care.
• Provide skilled nursing services, including medication management, wound care, and patient education.
• Monitor patient progress and communicate changes with physicians and the interdisciplinary care team.
• Coordinate care with therapists, social workers, and home health aides.
• Complete timely and accurate clinical documentation using WellSky.
• Educate patients and caregivers to promote safety, independence, and improved health outcomes.

What You'll Do

• Develop and implement individualized plans of care
• Provide skilled nursing services, including medication management, wound care, disease management, and patient education
• Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
• Coordinate care with therapists, social workers, and home health aides
• Complete timely and accurate clinical documentation using WellSky
• Educate patients and caregivers to promote safety, independence, and improved health outcomes
