Job summary

Human Touch Home Health is seeking an experienced Registered Nurse (RN) to join our team serving patients across Northern Virginia communities, including Fairfax and Falls Church. You will provide personalized, one-on-one home health nursing care, conducting assessments, developing plans, and educating patients and families to promote recovery and independence.

Qualifications

• Active Virginia RN license required.
• Driver's license and reliable transportation.
• Home health experience preferred but not required.
• OASIS experience preferred.

Responsibilities

• Perform comprehensive nursing assessments and skilled visits.
• Complete SOC/ROC/recertification/discharge assessments.
• Develop and implement individualized plans of care.
• Provide skilled nursing services including med management and wound care.
• Monitor patient progress and communicate with the care team.
• Coordinate care with therapists, social workers, and aides.
• Document clinically with WellSky in a timely manner.
• Educate patients and caregivers to promote safety and independence.

What You'll Do

• Perform comprehensive nursing assessments and skilled visits
• Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
• Develop and implement individualized plans of care
• Provide skilled nursing services, including medication management, wound care, disease management, and patient education
• Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
• Coordinate care with therapists, social workers, and home health aides
• Complete timely and accurate clinical documentation using WellSky
• Educate patients and caregivers to promote safety, independence, and improved health outcomes

Qualifications

• Active Virginia Registered Nurse (RN) License Required
• Valid Driver's License and Reliable Transportation
• Strong Clinical Assessment, Critical Thinking, and Documentation Skills
• Home Health Experience Preferred but Not Required
• OASIS Experience Preferred
• Experience with WellSky Home Health is a plus
• Passion for Delivering Exceptional Patient-Centered Care
