Job summary

Human Touch Home Health is seeking an experienced Home Health RN to provide personalized, one-on-one skilled nursing care in patients' homes across Maryland, including Prince George's County. This PRN role offers flexible scheduling and the opportunity to build meaningful patient relationships.

You will perform comprehensive assessments, develop plans of care, and coordinate with the care team to prevent hospitalizations and support independence.

Qualifications

• Active Maryland RN license required.
• Valid Driver's License and reliable transportation.
• Home Health experience preferred but not required.
• OASIS experience preferred.
• Experience with WellSky Home Health is a plus.
• Passion for delivering exceptional patient-centered care.
• Strong clinical assessment, critical thinking, and documentation skills.

Responsibilities

• Perform comprehensive nursing assessments and skilled visits.
• Complete SOC, ROC, Recertification, and Discharge assessments.
• Develop and implement individualized plans of care.
• Provide skilled nursing services, including med management, wound care, disease management, IV therapy as appropriate, and patient education.
• Monitor patient progress and communicate changes with physicians and the interdisciplinary care team.
• Coordinate care with therapists, social workers, and home health aides.
• Complete timely and accurate clinical documentation using WellSky.
• Educate patients and caregivers to promote safety, independence, and improved health outcomes.

What You'll Do
