Job summary

Human Touch Home Health in Washington, DC is seeking an experienced Registered Nurse (RN) for a PRN home health role. You will provide skilled, one-on-one patient care in patients' homes and contribute to care planning and education.

Ideal candidates have an active DC RN license, valid driver's license, and strong clinical assessment skills. This position offers flexible scheduling and opportunities for professional growth within an established agency serving the community for over 27 years.

Qualifications

• Active Washington, DC RN license required.
• Valid Driver's License and reliable transportation.

• Home health experience preferred but not required.

Responsibilities

• Perform comprehensive nursing assessments and skilled visits.
• Develop, implement, and update individual plans of care.
• Provide skilled nursing services including wound care and education.
• Complete SOC, ROC, recertification, and discharge assessments as appropriate.
• Monitor patient progress and communicate with physicians and care team.
• Coordinate care with therapists, social workers, and aides.
• Document clinically using WellSky and educate patients/caregivers.

What You\'ll Do

• Perform comprehensive nursing assessments and skilled visits
• Develop, implement, and update individualized plans of care
• Provide skilled nursing services, including medication management, wound care, disease management, and patient education
• Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments as appropriate
• Monitor patient progress and communicate changes to physicians and the interdisciplinary care team
• Coordinate care with therapists, social workers, and home health aides
• Complete timely and accurate clinical documentation using WellSky
• Educate patients and caregivers to promote safety, independence, and improved health outcomes

Qualifications

• Active Washington, DC Registered Nurse (RN) License Required
• Valid Driver\'s License and Reliable Transportation
• Strong Clinical Assessment, Critical Thinking, and Documentation Skills
• Home Health Experience Preferred but Not Required
• OASIS Experience Preferred
• Experience with WellSky Home Health is a plus
• Passion for Delivering Exceptional Patient-Centered Care
