Job summary

Human Touch Home Health seeks an experienced Home Health RN to serve patients across Northern Virginia in Fairfax, Falls Church, Alexandria, and surrounding areas on a PRN basis. You’ll deliver personalized, one-on-one nursing care in patients’ homes, build a caseload, and collaborate with a supportive clinical leadership team.

You’ll conduct assessments, care planning, wound care, disease management, and education to improve health outcomes while maintaining patient independence in the home

Qualifications

• Virginia RN license required.
• Valid driver’s license and reliable transportation.
• Strong clinical assessment, critical thinking, and documentation skills.
• Home health experience preferred but not required.
• OASIS experience preferred; WellSky experience a plus.

Responsibilities

• Perform comprehensive nursing assessments and skilled visits.
• Complete SOC, ROC, Recertification, and Discharge assessments.
• Develop and implement individualized plans of care.
• Provide skilled nursing services including medication management, wound care, disease management, and patient education.
• Monitor patient progress and communicate changes with physicians and the care team.
• Coordinate care with therapists, social workers, and home health aides.
• Complete clinical documentation using WellSky.
• Educate patients and caregivers to promote safety and independence.

What You'll Do

• Perform comprehensive nursing assessments and skilled visits
• Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
• Develop and implement individualized plans of care
• Provide skilled nursing services, including medication management, wound care, disease management, and patient education
• Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
• Coordinate care with therapists, social workers, and home health aides
• Complete timely and accurate clinical documentation using WellSky
• Educate patients and caregivers to promote safety, independence, and improved health outcomes

Qualifications

• Active Virginia Registered Nurse (RN) License Required
• Valid Driver's License and Reliable Transportation
• Strong Clinical Assessment, Critical Thinking, and Documentation Skills
• Home Health Experience Preferred but Not Required
• OASIS Experience Preferred
• Experience with WellSky Home Health is a plus
