Job summary

Human Touch Home Health is seeking an experienced Registered Nurse (RN) to join our growing DMV team, delivering compassionate, patient-centered skilled nursing in the home. You will perform comprehensive assessments, develop care plans, and coordinate with an interdisciplinary team to support recovery and independence.

As a Home Health RN, you’ll educate patients and caregivers, manage medications and wound care as appropriate, and help patients avoid unnecessary hospitalizations—all while

Qualifications

• Active RN license in Washington, DC, Maryland, or Virginia.
• Multi-state licensure or eligibility within the DMV preferred.
• Valid Driver's License and reliable transportation.
• Home Health experience preferred but not required.
• OASIS experience preferred.
• WellSky Home Health experience is a plus.
• Passion for delivering exceptional patient-centered care.

Responsibilities

• Perform comprehensive nursing assessments and skilled visits.
• Complete SOC, ROC, recertification, and discharge assessments.
• Develop and implement individualized plans of care.
• Provide skilled nursing services including medication management and wound care.
• Monitor patient progress and communicate changes with physicians and the care team.
• Coordinate care with therapists, social workers, and home health aides.
• Complete timely and accurate clinical documentation using WellSky.

What You'll Do

• Perform comprehensive nursing assessments and skilled visits
• Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
• Develop and implement individualized plans of care
• Provide skilled nursing services, including medication management, wound care, disease management, IV therapy as appropriate, and patient education
• Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
• Coordinate care with therapists, occupational therapists, speech therapists, social workers, and home health aides
• Complete timely and accurate clinical documentation using WellSky
• Educate patients and caregivers to promote safety, independence, and improved health outcomes

Qualifications

• Active Registered Nurse (RN) License in Washington, DC, Maryland, or Virginia required
• Multi-state licensure or eligibility to practice in multiple jurisdictions within the DMV is preferred
• Valid Driver's License and Reliable Transportation
• Strong Clinical Assessment, Critical Thinking, and Documentation Skills
• Home Health Experience Preferred but Not Required
• OASIS Experience Preferred
• Experience with WellSky Home Health is a plus
• Passion for Delivering Exceptional Patient-Centered Care
