Job summary

Human Touch Home Health in Montgomery County, MD seeks an experienced Home Health Registered Nurse (RN) for a PRN role serving patients across Bethesda and surrounding communities. You will deliver personalized, one-on-one skilled nursing care in patients' homes.

The role emphasizes independence, education, and advocacy, with opportunities to educate patients and caregivers, coordinate with a clinical team, and support recovery while keeping patients comfortable at home.

Qualifications

• Active Maryland RN License
• Valid Driver's License and reliable transportation
• Strong clinical assessment, critical thinking, and documentation skills
• Home Health experience preferred but not required
• OASIS experience preferred
• Experience with WellSky Home Health is a plus
• Passion for delivering exceptional patient-centered care

Responsibilities

• Perform comprehensive nursing assessments and skilled visits
• Complete SOC, ROC, recertification, and discharge assessments
• Develop and implement individualized plans of care
• Provide skilled nursing services including medication management, wound care, disease management, IV therapy as appropriate, and patient education
• Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
• Coordinate care with therapists, social workers, and home health aides
• Complete timely and accurate clinical documentation using WellSky
• Educate patients and caregivers to promote safety, independence, and improved health outcomes

What You’ll Do
