Job summary

Transitions Hospice in Springfield, IL is seeking a dedicated Night Nurse to provide professional nursing care during the overnight hours. You’ll deliver the full nursing process, assess patients, and support families with compassionate, high‑quality care in a home-like hospice setting.

You’ll coordinate with physicians, document care accurately, manage medications, and ensure equipment and services are in place for overnight patients.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a related setting preferred.
• Comfort and composure working independently during overnight hours.
• Strong assessment skills and sound clinical judgment in high-pressure moments.
• A calm, reassuring presence for patients and families during nighttime distress.
• Excellent documentation habits and communication skills.
• Valid driver's license and willingness to travel locally.

Responsibilities

• Delivering professional nursing care across the full nursing process within your scope of practice.
• Conducting thorough patient and family assessments and contributing to individualized care plans.
• Completing all visit types as needed to meet overnight patient and family needs.
• Administering medications and treatments per physician orders.
• Identifying nursing diagnoses from health assessment data and responding with sound clinical judgment.
• Recording accurate, timely clinical notes including pain and symptom management outcomes.
• Communicating changes in patient condition to physicians and obtaining orders as required.
• Coordinating with community health partners to support continuity of care.
• Ensuring equipment and necessary services are in place for patients.
• Maintaining and growing your clinical skills through continuing education and in-service participation.

Schedule: 4 - 10 hour shifts a week w/rotating weekends

Pay Range: $80,000 - $90,000 a year
